Temporary Food Service Permit

APPLICATION FOR A TEMPORARY FOOD SERVICE PERMIT FOR TOWN DAY

Please complete the following form to apply for a Temporary Food Service Permit through the Stoneham Board of Health.  If you would rather complete a hard copy, you can download the permit form here: Temporary Food Permit.

Please complete this form and submit all paperwork no later than Friday, September 1, to ensure the Board of Health has all the documentation necessary to approve your application. 

Please note:  Applicants must attest to reading  the Checklist for Temporary Events located here.

Application for a Temporary Food Service Permit

Temporary Food Event Permit - Operates for a period of no more than fourteen (14) consecutive days in conjunction with a single event or celebration, whether food is for sale or not. Application is due to Health Office 10 days prior to the event.

The following documentation is also required by the Stoneham Board of Health as it applies to your business in relation to this event. Please be aware, when certification is required, the certified person is to be onsite during the entire event to oversee safe handling of food.
